You don't need to fill out a claim form for VSP network providers. The doctor or provider will discuss any co-pays or out-of-pocket expenses with you during your appointment. They will submit a claim directly to VSP after your appointment.

To see if you are covered for out-of-network providers, call VSP Member Services at 800-877-7195.

If you went out-of-network and you have out-of-network coverage, you’ll need to submit a claim online or by mail.

To submit a claim, you will need a copy of the itemized receipts or service statements for each patient that includes the following information printed on them:

• Doctor’s name or office name
• Name of patient
• Date of service
• Each service received and the amount paid

You typically have 12 months from the date of service to submit for reimbursement.

1. Be sure your receipts have been scanned and are accessible by your computer
2. Log in to your vsp.com account
3. Click on View Your Benefits, then My Benefits
4. Scroll down and click Submit an Out-of-Network Claim
5. Complete the fields and follow the prompts
6. Upload your receipts
7. Click Submit

All claims must be submitted on a VSP Member Reimbursement Form. Contact VSP member services at 800-877-7195, or:

1. Log in to your vsp.com account
2. Click on View Your Benefits, then My Benefits
3. Scroll down and click Submit an Out-of-Network Claim
4. Complete the fields and follow the prompts
5. On step 4 of 4, click on Mail Receipts
6. Upload your receipts
7. Click Submit

After completing the claim form, you may print and mail copies of your claim form and receipt(s) to:

1. Contact VSP Member Services at 800-877-7195 and ask for a Member Reimbursement Form.
