LifeSecure Claims

Before submitting a claim, determine whether you have seen an in-network or out-of-network eye doctor or provider.

Claims Forms:

Online: Log in to the LifeSecure Portal and click “How to File a Claim.”

Be sure to gather the following information for the claims process:

  • Explanation of Benefits (EOB) from your primary insurance
  • Provider for each itemized bill
  • Copies of all itemized bills (may also be referred to as detailed invoice)
  • Medical notes and records (may also be referred to as Physician’s Report with notes)

Note: You don't need to wait until all EOB and itemized bills are received. Begin sending your claim documents as soon as possible.
