What is this insurance?
This plan pays a lump sum to you when diagnosed with a covered illness. The money is paid directly to you and is not based on the amount of money you have been billed or paid.
How much medical information do I need to provide?
For policy amounts $25,000 and under, you only need to answer question 1, parts A through G found in section 5 of your application. Any amount over $25,000 requires all questions answered.
How are deductions for my premium taken out?
Deductions are on a post tax basis. Because the plan is not paid on services performed, it does not fall under the medical insurance guidelines. With the deductions being taken out post tax, the benefit received is tax free.
